1.2 CREDIT REPORT

I understand the filing of a Chapter 13 or Chapter 7 bankruptcy will impact my credit score and will appear on my credit report for up to ten (10) years. The filing will be on my credit report even if my case is dismissed, voluntarily or involuntarily. My credit may be checked for future loans, applications for insurance and in other instances where credit checks are done.

I understand the decision to file a Chapter 13 or Chapter 7 bankruptcy is a personal decision and not a legal decision. I have been advised as to the legal consequences and I am making this decision of my own free will. I understand that BOND, BOTES, REESE & SHINN, P.C., cannot make this decision for me. I further understand that filing a Chapter 13 or Chapter 7 bankruptcy is a public record. As such, it is possible that my name may be listed in the local newspaper under bankruptcy filings and may be publicly available through the internet.
